In the practice of the Catholic Church, there are seven sacraments: Baptism, Reconciliation, Eucharist, Confirmation, Matrimony, Holy Orders, and Anointing of the Sick. A sacrament is an outward sign instituted by Christ in which we meet Him and He confers His divine life, and sanctifying grace, upon us. Through the sacraments, we respond to the Lord’s desire to be with us always and especially at significant moments on our journey of faith. The more intense our sacramental life, the greater is our Christian witness.
If you received the Sacrament of Baptism, or other sacraments here at St. Raphael, and would like a copy of the Baptismal (or other) Certificate, please complete the Sacramental Certificate Request Form (see link below). Please send the completed form to the Parish Office at the Church of St. Raphael. They may be emailed to attention of [email protected] OR [email protected] Please note that a copy of a valid driver’s license is required if the certificate is to be released to yourself. Otherwise, if the certificate is to be mailed directly to another parish, then the driver’s license copy is not required.
Also, please note that requests for certificates may be requested by the individual (if an adult), or by a minor child’s parent.
